Vantage ~ As H2O said MD2 is Multi-Displacement 2, it can export better normal map than using the original zbrush displacement exporter. MD2 can export multi texture with multi-UV placement, which means you are not limited to the 0-1 UV space in laying out UV map. And it is useful for complex and big model. But I haven't tried this yet.
I am using displacement map with base mesh of 8k polygons. As the low poly shape is having a big difference to the final sculpted shape, I have to export a mesh with a better shape and volume so that it can be properly displaced. The render time for the latest render was about 6 to 7 minutes on a P4 HT machine (I converted the displacement map from a 32-bit tiff to a MR .map file, it renders quick.)

For the MD2, I just look at the zbrush to maya doc only. So neither can I talk about the main operation of that.
